Service Details
Description:
Comprehensive assessment in the client's home by a multidisciplinary team, including a geriatric psychiatrist. Services include counselling, behaviour management strategies, health teaching, consultation and education for clients, families and caregivers/providers. Collaboration with family physicians. Linkage to support services.
Serve 34 LTCHs with this service.
Dr. Maxine Lewis: 2 days per wk
Dr. Jonathon Crowson – 1 day per wk
Dr. Rhonda Collins ( COE trained family physician – LTC patients with dementia and behave) 1 day per wk
Case Managers: 4.6 FTE

Criteria:
- Senior with complex mental health, addiction, and/or behavioural issue
- Not able to receive service in a traditional office or clinic setting
