Service Details
Description:
Comprehensive assessment in the client's home by a multidisciplinary team, including a geriatric psychiatrist. Services include counselling, behaviour management strategies, health teaching, consultation and education for clients, families and caregivers/providers. Collaboration with family physicians. Linkage to support services.
Dr. David Lam – 4.5 days per week
 Dr. Mariam Aziz – 1 day per week
 Dr. Dianne Giacomelli – 1 day per week
Case Managers: 11 staff/ 8.9 FTE

Eligibility Criteria
Criteria:
- Senior with complex mental health, addiction, and/or behavioural issue
- Not able to receive service in a traditional office or clinic setting
