Description:
Clinics provide specialized assessment and medical care for older adults with age-related disorders. Services are offered in the ambulatory clinic setting.
Treatment Approaches:
• Comprehensive Geriatric Assessment
• Collaboration with Primary Care MRP regarding diagnoses,
ongoing investigations, prescriptions, etc.
• Referrals for required testing
• Referrals and collaboration with community services
• Home visits are not available at this time
Note: Will serve patients under 65 years if experiencing difficulties usually associated with aging such as, Parkinson’s or Dementia.
Eligibility Criteria
Other Criteria:
Minimum Age:
65 yrs
Criteria:
- Must be a resident of Hamilton or the larger HNHB LHIN
- Any of the following clinical concerns:
• Neurocognitive disorders, memory concerns, dementia
• Mobility changes, gait disorders, falls
• Complex geriatric problems
• Multiple ER visits
• Polypharmacy
• Frailty or functional decline - Not currently seeing a Geriatrician elsewhere & has primary care physician
