During the COVID-19 pandemic HAL serrvices are extended to 5:30 pm last pick up
Service Details
Description:
Helps adults without immediate support to arrive home safely upon transitioning from hospital, and receive follow-up services at home. The service is up three hours on the day of transition and includes:
• Transportation home accompanied by a Personal Support Worker
During COVID-19 to maximize social distancing:
- Medication will be given to clients before discharge from hospital
- Comfort packages will be provided if requested
- frozen entrée for day of discharge
- grocery package available at low cost $10
• Help settling in at home from the Personal Support Worker
• Follow-up at home if not already connected to case management

No cost with exception of groceries at $10 a package.
