Description:
For Psychogeriatric Support
This program is a community step down program for individuals who have completed an outpatient mental health hospital program, or who reside in the community and wish for additional support in a group environment.
Program Highlights
A specifically tailored curriculum with an emphasis on mental health.
•Art, music, & life skills programming
•Support, case management, and education for clients and their families provided by our Social Worker
•Health promotion and disease prevention activities delivered our Nursing Staff
•Medication assistance
•Eating assistance and toileting provided by Program Aides
