Description:
A therapeutic. Program for older adults 55+. The program serves frail seniors and adults with physical disabilities or persons with early to
mid-stage Dementia / Alzheimer’s Disease.

Eligibility Criteria
Other Criteria:
Minimum Age:
55 yrs
Criteria:
- Independent or minimal assistance required. Referral from Ontario Health at Home required.
- Independent with toileting, and transferring with minimal assistance as required.
- Be mobile, but may use a cane, walker, or wheelchair with minimal assistance.
- Individuals must have appropriate social skills.
- Independent or minimal assistance with activities of daily living
- Not able to accommodate smoking during the program.
