Service Details
Description:
After Stroke is a personalized stroke recovery program that helps survivors and their families navigate the path forward after a stroke. The After Stroke coordinators work one-on-one with stroke survivors and their families to connect them with a personalized recovery plan, resources in their community, and a network of support to help navigate the journey ahead.
Once a referral has been received, the service user will receive a direct call and assigned to an After Stroke Coordinator.
