A senior's program that offers support 24-hours per day, 7 days per week with assistance to persons with light to moderate activities of daily living.

The SDL program is funded through the Local Health Integration Network.
Eligibility Criteria
Criteria:
The individual must be insured under the Health Insurance Act; be based on the eligibility criteria that is approved and funded by the LHIN; shall meet the characteristics of a high risk senior as set out by the MoH or unless otherwise directed by the LHIN; not be on a waiting list for a LTC housing facility unless otherwise specified by an approved external agency/health service provider; shall reside in a LHIN approved designated geographic service area but not reside in a care home; shall be be able to remain safely at home between visits unless otherwise negotiated by the LHIN and MODC; shall not require immediate 24-hour availability of nursing care or other professional services, and; the risk that a service provider who provides the service to the person will suffer physical harm while providing services must not be significant
