Description:

Offers one to one home and community support to adults with an acquired brain injury. Assists in home and community, gain previous skills and learn new skills to enhance quality of life and independence. The program works in consultation with a range of professionals on a consultation basis.

The program is funded through the Local Health Integration Network.
Eligibility Criteria
Other Criteria:
Minimum Age: 16 yrs
Criteria:
  • Client has a documented ABI.
  • Client requires support from MODC as a result of an ABI.
  • Client is willing to participate in a service that supports the individual to make informed choices based on their cognitive ability.
  • Client is not in receipt of funding IL services through a third party funder.
  • Client is is willing to participate in continued rehab services.
  • Client is unable to have his/her supports delivered through other existing community services.
  • Client has the ability to be safely left unattended for periods of time.
