Service Details
Description:

Program helps patients make the transition from hospital to home safely. Includes transportation home and support of a PSW from discharge through settling in once at home. Pick up of groceries, prescriptions if needed on the way home. Settling in includes ensuring safety, meal preparation, personal care, light housekeeping and laundry. Follow up following day by coordinator to ensure other services assist client to remain at home.
