This program based out of the Parry Sound location helps individuals who have had a stroke to get back to daily activities and live the fullest life possible.

A Stroke Community Navigator will work with the client to build an individualized plan for recovery; connect you to agencies that can offer the specific help they need, and access to educational group sessions developed by the Heart and Stroke Foundation.

Program offers on site exercise programs in group and individual formats, as well as monthly groups and lifeskills programs. All programs are individualized based on client goals and needs. The Navigator may accompany clients to appointments and help find transportation solutions.
