A primary behavioral health care integration program that provides medical care, care management and coordination, and peer support. The HILL Project follows the “Health Home” Model, which combines medical treatment with disease self management education, coordination of follow-up medical services, lab services, and peer-led wellness activities. The focus of HILL Project service delivery is achieving the “triple aim” of better care, improved outcomes, and lower costs.
