Description:

Home At Last (HAL): PSW staff assist clients to settle in their homes safely and comfortably after a stay in hospital. Services may include driving and/or accompanying the client home, picking up medications and/or groceries, preparing a small meal, providing personal care and/or homemaking services, follow up via telephone and/or a visit to check on the client's well-being, and referrals to other community support services.
Access to Primary Care (APC): Provides PSW accompaniment to high-risk older adults requiring support to attend their primary care appointments or assessments. PSW's will accompany clients to their appointment, provide support, and settle the client home safely following their appointment.

Eligibility Criteria

Criteria:

- Clients are aging, frail and/or have special needs, require additional settlement support upon discharge from hospital, and can be safely settled by one PSW.- Be a resident of Durham Region
