The Seniors at Risk System Coordinator provides support, consultation and access to supports for vulnerable older adults and their families in Guelph and Wellington County. This includes:
Coordinated community response to abuse of older adults with a collaborative, multi-disciplinary approach across health and social services, legal and police services.
* consultation and access to help for vulnerable older adults and their families, especially those who have experienced or are at risk of abuse
* considers wishes of the individual
* assistance to consider options
* provides referral to community resources/informal seniors groups when appropriate
* assists in accessing emergency shelter
