Service Details
Description:
Provides home visits to frail seniors to create a care plan for community supports and health care. Focus on those seniors discharged from hospital or who display complex needs.
IGSW can devote time to a senior to explain service options and be present when visits occur, to get services initiated.
Work closely with primary care, specialty care, community support services, and the Community Care Access Centre (CCAC) as partners in care.
