Description:

Service provided by Geriatricians, interdisciplinary case managers and nurses delivering evidence based, person and family centred care.



Treatment Approaches:
• Comprehensive Geriatric Assessment with ongoing case
management follow up
• Collaboration with Primary Care MRP regarding diagnoses,
ongoing investigations, prescriptions, etc.
• Referrals for required testing
• Referrals and collaboration with community services



Note: Patients under 65 years if experiencing difficulties usually associated with aging such as:
• . Parkinson’s or Dementia
• Osteoporosis
• Intellectual disability


Eligibility Criteria
Other Criteria:
Minimum Age: 65 yrs
Criteria:
  • Housebound
  • Not currently under care of geriatrician elsewhere
  • Any of the following clinical concerns:
    - Confusion
    - Memory Concerns
    - Dementia Diagnosis / Management
    - Functional Decline & Frailty
    - Falls / Instability
    - Mobility Changes
    - Bone Loss / Fragility
    - Complex Medication Review
    - Multiple Emergency Room Visits
