A geriatric psychiatrist and case manager provide specialized consultation, assessment, treatment and management to patients 65+ with moderate to severe stages of dementia, neurocognitive disorders or mental illness.

Minimum Age:
65 yrs
Criteria:
- Can safely attend clinic
- Risk of harm to themselves or others or exhibits challenging responsive behaviours not well managed by interpersonal or environmental interventions.
- A resident of the communities of West Hamilton (West of Ottawa St & West of Upper Gage Avenue) OR the larger HNHB LHIN territory if unable to access service closer to home
- Referral is from physician or NP
