Service Details
Description:

This Team provides specialized consultation, assessment, treatment and management of patients with moderate to severe stages of Dementia / Neurocognitive Disorders or Mental Illness who reside in Long Term Care Homes.



Treatment Approaches:
• Comprehensive Psycho-Geriatric Assessment with ongoing
case management follow up
• Collaboration with Primary Care MRP regarding diagnoses,
ongoing investigations, prescriptions, etc.
• Complex management of medications for severe Dementia
related behaviours
• Recommendations and collaboration with partners in the
development and monitoring of behaviour management care
plans
• Referrals and collaboration with community services



Note: Under 65 years if experiencing difficulties usually associated with aging such as, Parkinson’s or Dementia with associated behaviours

Eligibility Criteria
Other Criteria:
Minimum Age: 65 yrs
Criteria:
  • A diagnosis of any of the following:
    - Dementias / Neurocognitive Disorders
    - Mental Illnesses
  • Any of the following clinical concerns:
    - Risk harming themselves or others
    - Risk being aggressed by others
    - Exhibit challenging responsive behaviours not well managed by Interpersonal Interventions (IPI) and Environmental Interventions (EI)
    - Wandering and Exit Seeking
    - Complex Medication Review (Psychotropics)
    - Multiple Emergency Room Visits for Behaviours
  • Not currently under the care of a Geriatric Psychiatrist elsewhere
