Offers assistance with routine household activities such as light house cleaning, laundry, ironing, and preparing meals.

Provided at home
Provides temporary relief from caregiving role, reducing stress.
Personal Support Services
Provided at home
Help with a range of essential daily activities that help maintain physical well-being. Includes washing and bathing, mouth and hair care. Help with transferring to chairs, dressing, eating, and toileting.

Socializing & Outings Select a Category
Adult Day Program
Provides a daytime group environment of supervised activities, meals, companionship and recreation for older adults and those with chronic or palliative conditions.
Group Dining
Serves nutritionally balanced lunches, or dinners, in a social group environment.
Community Centre
Facilitates activities in a supported setting such as recreation, fitness, social, educational programs and events. Specialized programs in post-rehabilitation, chronic disease management, and nutrition education may also be available. Usually membership based.
Socializing (Virtual)
Provided at home
Provides education sessions for adults that focus on topics including basic literacy, computer skills, and resume development.
Support Group
In-person meetings where members provide each other with help for a particular life challenge. Often involves relating personal experiences and providing sympathetic understanding. May be clinician or volunteer-led.
Telephone Reassurance & Safety Checks
Provided at home
Provides regularly scheduled phone calls to ensure the safety for those with health issues or who experience loneliness or depression.
Virtual Exercise Programs
Provided at home
Provides exercise to improve cardio respiratory levels, muscular strength and balance. Available virtually wherever the client is, as long as they have access to the internet.

Education & Activity Select a Category
Health Education Classes
Provides education to participants to make choices that may improve wellness or reduce risks of disease. May focus on specific diseases or specific lifestyle issues (e.g. healthy eating).
Seniors Gentle Exercise Class
Provides group exercise classes, typically one hour in length, to improve strength and balance. Education provided to reduce risk of falls.

Food Bank (Mobile)
Provided at home
A place where stocks of food, typically basic provisions and non-perishable items, are supplied free of charge to people in need.
Case Management
Provided at home
Integrates health and social services by facilitating referrals and access to service across agencies. Supports care by providing follow-up after hospitalization or emergency room visits, or after specialist assessments.
Meals on Wheels
Provided at home
Delivers a nutritionally balanced noon hour hot meal by volunteers who provide a social contact and a safety check for the client. Special diets may be accommodated. Frozen meals may be available.
Rides & Transportation
Provided at home
Provides door-to-door rides for seniors or adults who have a disability to attend medical and therapeutic appointments, social activities and perform other daily tasks such as shopping trips.

Referrals to these services will first book an appointment with the Hub for service screening and coordination.
Assertive Community Treatment
Provided at home
These teams comprised of a multidisciplinary team, includes a mix of the following care providers: psychiatrist, nurse, case manager, occupational therapist, social worker, addictions worker, and/or peer support workers.
Drop in & Group Programs
Provided at home
Community-based programs that include employment and education support, social recreation, meal programs, drop-in groups, peer support and accompaniment services. Many of these programs can be accessed anonymously.
Mental Health Case Management
Provided at home
Programs provide one-to-one supports to individuals in identifying goals that will help them meet their health care needs. Services include assessment, planning, supportive counselling, assistance with activities of daily living,

Community Counselling or Treatment
Provided at home
Programs provided with the goal of assisting individuals with developing skills to manage mental health, substance abuse and related problems, and/or maintain and enhance treatment goals.
Opiate Outreach Treatment & Counselling
Provided at home
Community-based programs that include counselling and therapy, opioid agonist treatment, health education, and outreach services to individuals currently using opiates and/or methadone or Suboxone/buprenorphine.
Residential Treatment
Provided at home
A highly-structured treatment program designed to equip clients with the tools they need to make sense of the effects of problematic substance use or dependence and its resulting behaviours, emotional impacts, and ambivalences.
Withdrawal Management
Provided at home
Withdrawal support to individuals in an acute state of intoxication or withdrawal from alcohol or other substances.
Crisis Services
These services are available for clients to receive immediate emergency interventions during a crisis.
Telephone Helpline
Provided at home
A telephone service to provide information and support.
Crisis Outreach Services for Seniors
Provided at home
On-call mobile crisis intervention and outreach service for older adults (65+ years) with addictions issues or mental illness.
