Description:
Traverse Independence provides supportive housing and personal support services for adults with physical disabilities. Clients live in their own apartments and are responsible for their own rent and living expenses. We provide support services 24 hours a day/ 7 days a week, which are scheduled on a daily basis with additional on-call services as required.
Support services may include Attendant Service Workers, Life Skills Facilitators and Behavioural Facilitators. We support clients in many areas, including money management, personal care, and interpersonal relationships.
Attendant services may include: diabetic care/medications, bowel and bladder routines, prescribed exercises, ventilator care, tube feeding, bathing and grooming, dressing, light homemaking, meal preparation.

Cost to Client:
While the attendant services offered at each location are fully funded through the Ministry of Health & Long-Term Care, consumers are responsible for all other expenses including rent, utilities, food and transportation.

Criteria:
To be eligible for our supportive housing services, applicants must:- Be a least 16 years of age- Have a valid Ontario Health Card- Have a permanent physical disability- Require a minimum of 3 hours of service per day- Be medically stable- Not have active addictions or mental health issues- Demonstrate the ability or potential ability to direct their own service- Not be dangerous to themselves or others
