Description:

A significant injury challenges your ability to reclaim your independence. Whether you are coming from the hospital or from living with caregivers, the first step is a 12-week assessment in a fully furnished apartment. It includes heat and hydro, cable, local phone and internet and is equipped with basic kitchen wares and everything you need, except your clothes, toiletries and personal items. We provide a weekly allowance to buy your groceries items, cleaning supplies and other items. Public transportation passes are included if needed.
Once an independent living skills assessment has been done, you and your team will establish which skills you need in order to accomplish your personal goals. The plan is designed to take you to independence in the community with the appropriate supports.

Minimum Age: 16 yrs
Criteria:
- Have a valid Ontario Health Card- Have a brain injury diagnosed by a regulated health professional- Be medically stable- Demonstrate the desire to be involved in programs- Not be dangerous to themselves or others- Not have severe behavioural problems- Not have ongoing psychiatric or substance abuse issues
