We ensure you or your loved one receive
- A personal assesment to identify needs, concerns and risks
- A Care Plan which is developed in partnership with you and if needed, your caregiver
- Referrals to community resources or programs, in-person counseling or telephone support counseling;
- Care which empowers you or your loved one to be in control of your health, by enhancing decision making skills and providing support to develop effective coping strategies;
- Client advocacy;
- Crisis intervention
