Description:
A team of nurses, occupational therapists, social workers, and other professionals provide health care and personal support at home for a variety of reasons including: hospital transition, illness or injury, and complex or chronic health conditions. The care coordinators work with the patient to determine their needs and a customized care plan that the team will carry out.
