Provides the patient the ability to take charge of their care using technology and remote monitoring to send your health checks to your telehomecare nurse and learn to manage their condition. The nurse provides weekly health coaching, is in direct communication with the patient's primary care provider, and will respond as needed if health checks fall out of normal range.
