Description:

Personal Support Workers are available 24 hours a day, seven days a week, to assist residents with the activities of daily living as assessed and consented to by the resident, caregiver/substitute decision maker, and Assisted Living Supervisor.



Residents benefit from knowing care is available on an as-needed basis.
Caregivers and families benefit through the knowledge that their family member or friend is cared for on a daily basis.



Assistance can include:
1. Personal care

2. Safe transfers
3. Medication reminders (blister pack required)
4. Laundry
5. Referral services
6. Essential housekeeping
7. Security checks
8. Light meal preparation/meal warming
9. 24-hour emergency response within established time frame
10. Alternate level of care planning and linking

Cost to Client:
No charge for eligible residents.
Eligibility Criteria
Criteria:
High Risk Seniors who are currently not on a waitlist for LTCH admission and who are designated eligible for the service by the CCAC based on the eligibility criteria as set out in the Assisted Living Services for High Risk Seniors Policy, 2011. Seniors whose needs are best met with short, frequent visits and/or who need assistance at any time throughout the day.
