Description:
A community subsidized based program aiming to meet the needs of the frail elderly through a personalized plan of service. This program includes a comprehensive FREE Home Safety Assessment where recommendations are made to help improve the quality of life for older adults who wish to maintain their independent living arrangements.
Home Adaptations include:
• Bathtub cutouts
• Grab bars
• Handrails
• Raised toilets and bathroom support equipment
• Improved lighting and more
Home Maintenance includes:
• Spring/fall yard clean-up
• Snow removal
• Summer lawn care
• Eaves trough cleaning
• Exterior window cleaning

Client is assessed as having difficulty with daily living activities due to age-related loss.
