Description:
Weekly consultation between Primary Care providers at Kitchener Family Practice Inc. and Waterloo Wellington HCCSS Care Coordinator to facilitate collaborative approach to care and share information to enhance care.
Primary Care can add a patient to the weekly consultation list to discuss care needs and/or update HCCSS on the patient's health status, whether or not the patient is already connected to HCCSS.
HCCSS can add a patient to the weekly consultation list to update Primary Care on the patients care needs or health status.
