Description:
If you have a patient with an acute or chronic care need that is not met with current treatment and supports, we can can support them at home and in the community.
A helpful HCCSS staff member will contact the patient and find out more information about how we can help them in his or her home.
HCCSS offers in-home nursing, occupational therapy, physiotherapy, dieticians, speech language pathology and personal support.

Eligibility Criteria
Criteria:
Valid Ontario Health Card. Adults over 16 years of age with an acute or chronic health condition.
