Care Coordination - On Site Integrates health and social services by facilitating referrals and access to service across agencies. Supports care by providing follow-up after hospitalization or emergency room visits, or after specialist assessments.

Court Support Services Provides short-term case management for individuals with mental health and/or substance use concerns who are involved in the criminal justice system. Includes help navigating court processes, meeting legal conditions, and connecting to community services.

Employment Counselling Assistance with a plan of action to achieve individual career and employment goals.

Social Activity Social programs that help keep people engaged with their friends or community. Activities are usually 1-3 hours and are often recreational. Programs may be offered virtually.
