Description:

Intended for people with severe and persistent mental illness who would otherwise rely heavily on inpatient psychiatric hospital services. Clients can access the service for as long as they need.



This program is delivered by a multidisciplinary team. Each client receives services from all members of the team, working collaboratively to develop an individualized treatment plan. ACTT provides; psychiatric treatment, including medications, continuity of care, nursing, regular contact with a team member, collaboration with families and other community agencies, recreational support, peer support, addiction counseling, consultation with a registered dietician, occupational therapy

Minimum Age: 16 yrs
Criteria:
  • Client must be a resident of S-LLG (south of Highway 7)
  • Client must have one or more of the following problems:
    ○ High use of Schedule 1 hospital services or specialty hospital services, tertiary level services, or psychiatric emergency services such as mental health crisis response services
    ○ Intractable (i.e. Persistent or very recurrent) severe major symptoms (e.g., effective, psychotic, suicidal)
    ○ Coexisting substance abuse disorder of greater than six months ○ Involvement with the criminal justice system due to mental health disorder, assessed at low to moderate risk in the community , and the ACT team has determined that it is able to manage the current level in the community.
    ○ Inability to consistently meet basic survival needs, residing is substandard housing, homeless, or at imminent risk to becoming homeless
    ○ Residing in an inpatient or supervised community residence, but clinically assessed as being able to live in a more independent living situation if intensive services are provided, or requiring a residential or institutional placement if more intensive services are not provided
  • Client must be 16+ years of age
