Provides vision assessments, mobility, and life skills training and accessible library services. Recommendations for technical aids and computer adaptations.

Fitness Group
Exercise sessions where participants develop physical fitness and/or skills within a defined sport. May be either beginners or advanced.
Friendly VisitingProvided at homeOffers no charge visiting companionship such as conversation, reading, games, or crafts, while providing a safety check. The contact may be face-to-face at the client's home or through a phone call.
Health Education Classes
Provides education to participants to make choices that may improve wellness or reduce risks of disease. May focus on specific diseases or specific lifestyle issues (e.g. healthy eating).
Home Support HelperProvided at homeProvides help in the home such as meal preparation and planning, light housekeeping, companionship, errands, medication reminders and assistance getting to outings.
HousekeepingProvided at homeOffers assistance with routine household activities such as light house cleaning, laundry, ironing, and preparing meals.
Meals on WheelsProvided at homeDelivers a nutritionally balanced noon hour hot meal by volunteers who provide a social contact and a safety check for the client. Special diets may be accommodated. Frozen meals may be available.
Nursing (In-Home)Provided at homeNursing care for a chronic or temporary condition. Hands-on care is offered (e.g., wound management, IV therapy) and teaching and monitoring. (e.g., diabetic teaching) Available on a shift or visit basis.

Personal Support ServicesProvided at homeHelp with a range of essential daily activities that help maintain physical well-being. Includes washing and bathing, mouth and hair care. Help with transferring to chairs, dressing, eating, and toileting.
Physiotherapy (In-Home)Provided at homeService may include education, physical manipulation, massage, hydrotherapy, and exercise therapy, to assist recovery of damaged tissues. Delivered at client's home.
Rides & TransportationProvided at homeProvides door-to-door rides for seniors or adults who have a disability to attend medical and therapeutic appointments, social activities and perform other daily tasks such as shopping trips.
Social Work (In-Home)Provided at homeSocial work provided in home setting.
Speech-Language Rehabilitation (In-Home)Provided at homeIdentification, evaluation, and treatment of acquired communication disorders such as difficulty understanding or producing speech and swallowing. (Provided at home.)
Transitional Care Facility
Provides nursing and rehabilitative care for patients being discharged from hospital. (E.g., for rehabilitative care due to fractures and joint replacements, wound care, stroke recovery.)
