Service Details
Description:

Provides a comprehensive assessment of older adults with complex cognitive, physical, social, psychological and environmental needs.



It promotes independence & the well-being of the older adults by supporting them & their caregivers through diagnosis, treatment and health teaching within a holistic, comprehensive & coordinated care approach.



Treatment Approaches:
- Consultation & follow-up with geriatricians in out-patient clinics.
- Case-management by nurse clinicians.
- Collaboration with community partners for coordinated care.



Note: Adults under the age of 65 with complex concerns will be reviewed & considered on an individual basis (i.e. early onset dementia).


Eligibility Criteria
Other Criteria:
Minimum Age: 65 yrs
Criteria:
  • Adults who are frail or have multiple complex problems, such as but not limited to:
    - Cognitive impairment
    - Frailty
    - Falls / Mobility issues
    - Polypharmacy
    - Frequent hospitalization &/or ED visits
    - BPSD
    - Mood disorders
    - Caregiver burnout
  • Not in a long-term care facility.
