Description:
The team works with patients and their families to set achievable goals, which are based on individual needs, and monitored on a daily basis. Through daily therapy and active participation patients are able to track their progress toward these goals.

There may be some costs, such as an assistive medical device, that are not covered by OHIP when a patient is receiving rehabilitation services and care. These costs are the responsibility of the patient and their family.

You must have a valid Health Card and a doctor's referral to be eligible.
