This is a 30 minute phone appointment booked with a team member to register clients for our education, social/recreational programs, help navigating the community support system or counselling support (individual/family/group).
Please note - we also have a French speaking team member for individuals who prefer to speak in french.

The Alzheimer Society Waterloo Wellington provides for the Person Living with Dementia and/or their Care Partner:
• System Navigation
• Education Sessions and Resources
• Individual/Family Counselling/Support with Registered Social Workers
• Therapeutic/Support Groups with Registered Social Workers
• Social/Recreation Programs (including Minds in Motion)

To reduce your client’s wait-time, please share the New Client Registration form with your client at the time of the referral.
