Description:

St. Joseph's Adult Day Programs are for older adults, living in the community, alone or with family, who would benefit from a wellness oriented program, including social recreational activities, monitoring and assessment in a supervised, therapeutic environment. The day program also provides caregiver relief, education for families and individuals, support and advocacy. Here is an example of our Calendar: http://www.sjhcg.ca/out-n-about



Program is NOT suitable for individuals:
- who require two person transfers
- who require 1:1 support
- who require the use of a mechanical lift
- who require feeding assistance

Available from: Transportation is NOT available through the day program. Other organizations include VON, Guelph Mobility, private taxi's, Just Call Lynda

Eligibility Criteria
Criteria:
  • Older adults with a physical disability, and/or frailty. Older adults experiencing social isolation, loneliness or has a mental health diagnosis who reside in community (alone, assisted, caregiver respite). Does not live in LTC facility.
  • Be able to independently feed themselves.
  • Physically and behaviourly stable and does not require 1:1 ongoing support from staff.
  • Able to transfer and receive care with a maximum 1 person assist (unable to support the use of transfer boards).
  • Able to provide an emergency contact who is able to pick up the participant from the program within 45 minutes of a call, if the need arises (ie. participant becomes ill, or can not be safely supported in the program).
  • Aware that SJHCG is a smoke free facility, and smoking can not be supported while in the day program unless the individual can independently go off the property and return.
  • Is not considered to be at risk of wandering or eloping from program environment (unsecured environment).
  • Able to provide an email address for the participant or the caregiver to receive the referral confirmation (no longer able to provide telephone confirmation).
