Description:
Optimizes the frail senior's level of independence. Individual goals are established for each patient and weekly assessments evaluate the patient's status in relation to the established goals. These may include personalized education plans and links with community services.

Eligibility Criteria
Criteria:
This program is appropriate for patients with:- Mild cognitive decline with multiple co-morbidities- Physical/Functional decline (i.e. polypharmacy, falls)- No serious/persistent mental health illness complicated by aging, mild cognitive issues, comorbidities- No definitive cognitive-related diagnosis (requires an initial/full assessment of cognitive impairment)- Not a risk to self or others- A clear diagnosis - All abnormal lab values have been acknowledged and addressed- Acute medical issues have been resolved or reached a plateauExclusion Criteria:- Those exhibiting physically responsive behaviours- In need of high flow oxygen (>4L/min)- Exacerbation of a chronic psychiatric diagnosis- Formed patientsAll community resources must have been exhausted
