Description:
For Partner Sites within the Caregiver Social Prescribing Program: Connecting caregivers with support through the local Caregiver Link Worker of The Ontario Caregiver Organization. Please ensure the person you are connecting is a caregiver (caring for a family member, partner, friend, or neighbour) residing in the NORTHEAST region of Ontario and you have their consent to complete this form.
A Caregiver Link Worker will contact the caregiver within 3-5 business days from the date of the social prescription.

- Is caring for a family member, partner, friend or neighbour in the Northeast of Ontario?
- Caregiver living in one of the following communities?
-Goulais Bay
- Heyden
- Sault Ste. Marie
- Echo Bay
- Laird Township
- Desbarats
- Bruce Mines
- Thessalon, including the First Nations of Batchewana
- Garden River
