he model is a consultative service for professionals and individuals (clients/patients/families) that are living with diabetes. The diabetes team helps the individual to self-manage their diabetes.

The interprofessional team consists of certified diabetes nurse educators and certified diabetes dieticians.

The Paediatric team also has a paediatrician and social worker.

Referral to the Diabetes Program is by physician or self-referral from the client/patient/family by calling the diabetes program.
