Eligibility Criteria
Criteria:
- Is the senior 75 years of age or older?
- Is the senior in need of assistance with daily living due to functional or cognitive impairment?
- Is the senior at risk for hospitalization or long-term care home admission?
- Is the caregiver unable to assist or is there a risk of caregiver burnout?
- Does the senior’s income fall below the financial threshold? Individual under $34,999 and couple under $49,999.
