Service Details

A Nurse Practitioner specialized in Acquired Brain Injury (ABI) will provide the following services:
- Communicate with primary care on a regular basis
- Complete ABI specialized assessments with input from specialist physicians accessed via OTN
- Develop a support plan and support client to achieve goals
- Provide opportunity for consultation with an ABI specialized interdisciplinary team

Minimum Age: 18 yrs
Adults who:- have a diagnosed acquired brain injury- are experiencing significant issues that are not being managed through existing ABI services- have completed the acute phase of recovery- have a valid Ontario health card.
