After Stroke is a personalized stroke recovery program that helps survivors and their families navigate the path forward after a stroke.

Once you have submitted a service request, your local After Stroke coordinator will contact you. They will work with you to understand your needs and goals for stroke recovery, and develop a personalized plan to help you reach them. The plan is regularly reviewed and adjusted. They will help you connect to support, educational opportunities, programs, and other stroke survivors who have been in your shoes.
