              Reside in the following areas: New Sudbury, Garson, Minnow Lake, Coniston, Valley/Capreol. 
Have a need for support with personal care routines in their home; have the ability to self-direct their own care to the eligibility level of the service; be a resident of Ontario and insured under OHIP, and; not be in receipt or expecting to be in receipt of insurance funds for the provision of these services.
