Description:
Provides Assertive Community Care supports which may include a transitional bed program (if applicable) for clients with Bariatric and/or complex Mental Health needs.

Other Criteria:
Minimum Age:
18 yrs
Criteria:
- Weight > 400 lbs; BMI greater than or equal to 40 (flexible)
- BMI greater than 30 but less than 40 with one of the following co-morbidities:
i. Hypertension
ii. Type II diabetes
iii. Coronary Heart Disease
iv. Obstructive Sleep Apnea Syndrome
v. Gastro-esophageal Reflux Disease - Participation from the patients in the following areas (at least one area):
i. Weight loss goals
ii. Dietary restriction in the program
iii. Mental Health Support - Passive and Active rehabilitation
- Destination in the community
- Will benefit from the following allied health therapy services (e.g. OT, PT, PTA/OTA, RD or Recreation Therapy)
- Activity tolerance for daily group and/or 1:1 sessions for 30 minutes to 2 hours per day (5-7 days per week)
- Meets IPAC transfer criteria (as per TGHC’s protocol)
- Patient behaviours are manageable
- Bed bound patients will be considered if meet other criteria
