Offered in partnership with other Independent Living Service providers to enhance successful community reintegration after one has experienced a stroke through Stroke Community Navigation Services and rehabilitation services offered both one-on-one and in a group setting.

In addition, the program offers to qualifying individuals a short sty transnational apartment to assist with transition from hospital to home.
