              Description:
            
            Qualified Attendant Care staff provide assistance with activities of daily living in an integrated apartment building. Services include, but are not limited to, assistance with personal hygiene, rising and retiring, meal preparation and toileting.

              Eligibility Criteria
            
          
                  
          
              Criteria:
            
            
              The individual must be insured under the Health Insurance Act; be based on the eligibility criteria that is approved and funded by the LHIN; shall meet the characteristics of a high risk senior as set out by the MoH or unless otherwise directed by the LHIN; not be on a waiting list for a LTC housing facility unless otherwise specified by an approved external agency/health service provider; shall reside in a LHIN approved designated geographic service area but not reside in a care home; shall be be able to remain safely at home between visits unless otherwise negotiated by the LHIN and MODC; shall not require immediate 24-hour availability of nursing care or other professional services, and; the risk that a service provider who provides the service to the person will suffer physical harm while providing services must not be significant
