The South County Mental Health Center’s assertive community treatment team responds to those mentally ill clients who have demonstrated a high need for services, which are provided as intensively as needed.

Our case managers on the team deal with a smaller-than- usual caseload so that they can focus on these intense-need clients with the goal of preventing hospitalization and sustaining the client in the community.

To ensure success, the SCMHC treatment team coordinates a client’s care with other providers to ensure optimal support and services in the realms of physical health, social services, vocational training, residential services and economic support.
