Description:
A referral program that provides seamless integration between the cessation services of Smokers' Helpline and healthcare providers who identify and refer patients or clients who smoke or want to remain tobacco-free.
The Quit Connection Program eliminates the need for patients or clients to call Smokers' Helpline. Instead, a Smokers' Helpline Quit Coach will contact the tobacco user to provide counseling.
Upon receipt of the referral and a call is made to client at the indicated preferred time. Four attempts will be made to contact the client. Tobacco Cessation Specialists (Quit Coaches) provide information about the service, assess a client’s readiness to quit, and offer services based on each individual client’s situation

Criteria:
Any healthcare provider in Ontario, Saskatchewan, Manitoba, New Brunswick and Prince Edward Island can access Quit Connection in order to refer patients or clients to Smokers’ Helpline
