Description:

The Going Home Program aims to help seniors and adults with a dissability to remain safely at home after leaving hospital.
The program offers community support services free of charge for the first 10 business days from when individuals arrive home from hospital.



Services may include:



 Transportation from hospital to your home upon discharge
 Transportation to medical follow-up appointments
 Home help such as laundry and light housekeeping
 Personal care such as help with bathing
 Meals on Wheels delivery of hot or frozen meals to your home
*Service limits apply



Services will not be duplicated either through Home and Community Care, new or existing services in the community, or paid services.

Minimum Age: 65 yrs
Criteria:
  • Seniors or persons with disabilities returning home from hospital stay.
  • Mobility aligns with Champlain Community Transportation Collaborative criteria.
  • Clients living in Retirement Homes are eligible for transportation services only.
  • Clients with contact precautions are eligible dependent on the availability of PPE for client and driver (if necessary), and referral includes a description of the precaution. Intake reserves the ability to refuse the referral if deemed unable to safely service the client.
