Description:

The Supportive Housing Program provides non-medical assistance with the activities of daily living and can include assistance with any or all of the following activities:
* personal grooming and hygiene
* bathing or showering
* dressing and undressing
* toiletting
* rising and retiring
* meal preparation and assistance with eating
* positioning and transferring
* medical procedures as deemed appropriate by a professional community health care provider

Funding for the Supportive Housing Program is provided with the Hamilton Niagara Haldimand Brant LHIN
Eligibility Criteria
Criteria:
To be eligible to receive Supportive Housing Services the applicant is:- a resident of Ontario- insured under the Insurance Act of Ontario- safe in his/her own home when left unattended- able to direct his/her own personal support and homemaking services- determined through participation in assessment to be physically unable to safely complete the routine activities of living within a reasonable time- able to have necessary medical needs met by the existing community health care network
