Description:
This program helps you understand your health risks and take steps to stay well. A Prevention Specialist will talk with you about your health, lifestyle, and any challenges you face. Together, you’ll create a simple preventive care plan that may include cancer screening support, diabetes risk checks, healthy living coaching, or help finding a primary care provider. We can also connect you to community programs and services. This service is free and available to adults of all ages.
